Lost Operator Cards (Lost Boater card)
If you took your boater test in Ontario or any other Canadian province, then you may obtain a replacement Operator Card from the same provider but you must remember the company you wrote the exam with.

The ministry does not keep a master database so the other option may be to re-write the exam online here.
